Pareto Analysis of historical lagging and real-time indicators is used to identify where effort is most needed and most likely to be effective.  Corrective actions are then identified and turned into SOPs, and staff trained in their implementation.  Performance of the SOPs is then measured and analyzed against lagging and real-time indicators for the new period.  From analysis of this data, most effective corrective actions are identified along with any problems in implementing proposed corrective actions.  Where more than one unit are implementing the same corrective actions, the actions of the different units are analyzed to assess whether there are transferable lessons from the unit with the best performance.  This process is iterated to ensure continual improvement.

In order to improve its safety continuously, an organization must measure what it is doing and ascertain what actions most contribute to both safety and to undesired outcomes.  For this, the organization needs to:

  • know the events and outcomes it needs or wants to avoid
  • measure the incidence of such outcomes (lagging indicators)
  • measure the incidence of actions which give rise to the unwanted outcomes (real-time indicators)
  • measure the actions which prevent actions which give rise to unwanted outcomes (leading indicators)
